Crawford County Mental Health Center in Kansas seeks a Licensed Bachelor Social Worker (LBSW) to provide community-based behavioral health services, case management, crisis intervention, and coordination for children, adolescents, and families experiencing emotional, behavioral, mental health, and substance use challenges.

You will deliver goal-directed interventions in home, school, and community settings.

Qualifications

Bachelor's degree in Social Work required.
Current Kansas Licensed Bachelor Social Worker (LBSW) license preferred.
Ability to secure Kansas Bachelor's Level Social Worker licensure within 60 days of employment.
Experience with children, adolescents, and families preferred.
Experience in community mental health, crisis services, school-based services, or behavioral health settings preferred.
Valid Kansas driver's license and acceptable driving record required.

Responsabilités

Provide regularly scheduled social work encounters and rehabilitation services per treatment plans and IEPs where applicable.
Document all services in the school system and EHR on the same day.
Assist clients in implementing tasks related to treatment plan goals.
Facilitate psychosocial rehabilitation groups and social skills development.
Provide individual social, emotional, behavioral, and problem-solving skill development services.
Monitor progress toward goals and adapt interventions as needed.
Coordinate goals with parents, school personnel, and providers.
Collaborate with school staff, therapists, and community agencies for continuity of care.
Assist families in accessing community resources and transportation when appropriate.
Advocate for clients to ensure access to services and supports.
Respond to crises with crisis intervention and de-escalation under QMHP direction.
Coordinate referrals, transitions of care, discharge planning, and follow-up.
Deliver services in homes, schools, and agency facilities.
Maintain compliance with policies, Medicaid, HIPAA, and regulations.
Participate in meetings, supervision, training, and QA activities.
Report to Youth Crisis Stabilization Unit or Challenger program when school is out; assist with Challenger groups on school days.
Perform other duties as assigned.
