• Knowledge of laws, regulations and practices pertaining to federal and state public welfare programs.
• Knowledge of federal, state and local resources, their organization and the ways in which these resources can assist individuals and families.
• Knowledge of living conditions, values and behavior of the ethnic and subcultural groups served by the agency.

• Provides primary case management for assigned clients, including assessment, service planning, monitoring, and evaluation.

• Provides consultation to human service agencies.
• Provides community education to the public and various agencies.
• Completes and maintains necessary documentation and reports according to applicable requirements, codes and policies.

Our Behavioral Health Unit is looking for compassionate, energetic and goal-oriented individuals to join our team today!

Under supervision of a Behavioral Health Supervisor provides referral, case management, and direct services to , adults and the elderly with specific issues including abuse and neglect, chronically ill and/or disabled, mentally ill and alcohol and other substance abuse.

Screens and evaluates individuals to determine appropriate services based on individual client needs.

Develops and implements service plans that provide appropriate services to the client and their families.

Provides primary case management for assigned clients, including assessment, service planning, monitoring, and evaluation.

Provides direct community based services to specific client populations.

Participates in discharge planning from inpatient or nursing home placements.

Provides consultation to human service agencies.

Provides community education to the public and various agencies.

Completes and maintains necessary documentation and reports according to applicable requirements, codes and policies.

Accepts requests and referrals for unit services.

Recruits volunteers by utilizing media, speaking engagements and other methods.

Assesses and investigates reports or referrals of abuse and neglect of adults.

Makes collateral contacts with medical, legal, mental health and other human services agencies and organizations; provides information and referral services.

Performs related functions as assigned.

Bachelor's degree in social work or related human services field; experience in social service agency

Licenses and Certification:

Valid Wisconsin Driver's License

State Social Worker Certification preferred
