Qualifications

• Master’s Degree in healthcare or related field required.
• 3-5 years of Behavioral Health experience preferred.
• 1-3 years of Behavioral Health Managed Care Case Management experience.
• Knowledge of NY State Health Home System required.
• Bilingual preferred.

Responsibilities

• Clinically assess members for health needs and document care plans.
• Meet with members in various settings and document care coordination notes.
• Attend interdisciplinary care team meetings and optimize quality of care.
• Educate providers and assist with housing needs.
