Job summary

MetroPlusHealth is dedicated to empowering New Yorkers by uniting communities through care. The Care Manager role focuses on optimizing members’ health care experiences, assessing needs, coordinating services with the care team, and developing individualized plans to improve outcomes.

Based on NY regulatory standards, you will engage with patients in clinical or community settings, conduct psychosocial evaluations, arrange referrals, and document progress.

Qualifications

• One year of experience in case management, care coordination, discharge planning, utilization management/review, QA/management, or related area in a health or managed care setting.
• Doctoral degree in Psychology or related discipline, or one of the substitute qualifications may apply.
• Licensure: LMHC, LMSW, LCSW, or CASAC as applicable in NYS.
• For behavioral health roles, three years of experience and valid license as LMHC/LMFT/LMSW/LCSW per NYSED.

Responsibilities

• Screens charts and conducts psychosocial evaluations; formulates treatment plans.
• Provides individual and group counseling in health care or community settings.
• Collaborates with care team to develop and implement multidisciplinary treatment plans.
• Assists in discharge planning and post-acute care arrangements as needed.
• Documents and reports progress; ensures compliance with policies and regulations.
