Job summary

Envision Health Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join the Assertive Community Treatment (ACT) Team in Virginia.

You will provide community-based nursing, medication management, and support to adults with serious mental illness within a multidisciplinary setting. The role emphasizes collaboration with RNs and psychiatric providers, adherence to Virginia DBHDS/DMAS/TMACT standards, and involvement in daily team meetings, care coordination, and crisis response as

Qualifications

• Active LPN license in Virginia required.
• Minimum 1 year experience in behavioral health, psychiatric nursing, community health, or substance use services preferred.
• Strong communication, organization, and interpersonal skills.
• Ability to work effectively within a multidisciplinary treatment team.
• Valid Virginia driver's license with reliable transportation required.

Responsibilities

• Provide community-based nursing services and support to ACT participants.
• Assist with medication management, monitoring, and education.
• Administer medications and injections under supervision of RN or treating provider.
• Monitor treatment effectiveness, side effects, and overall wellness.
• Assist clients in understanding diagnoses, treatment plans, and medications.
• Coordinate with pharmacies and providers regarding medications and healthcare needs.
• Participate in daily ACT team meetings and treatment planning.
• Conduct wellness checks and medical appointment follow-up.
• Assist with crisis intervention and stabilization as directed by ACT team.
• Support transitions from hospitals or other placements.
• Maintain accurate documentation in the electronic health record (EHR).
• Ensure confidentiality and HIPAA/DBHDS/DMAS compliance.
• Participate in after-hours on-call rotation.
