Job summary

Clarity Care is seeking an Activity Support Provider for the Day Services Program in Green Bay to help members with daily living activities, personal cares, and community involvement. You will assist with goal setting, independence, and safeguarding consumer rights while promoting dignity and respect in all interactions.

Responsibilities include planning and leading engaging activities, preparing meals per dietary needs, and supporting members in achieving personal goals.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

• Assist members with physical, social, and personal needs
• Guide members in achieving personal goals and decision‑making
• Create fun and engaging activities tailored to member interests
• Prepare and serve meals following dietary needs and preferences
