A Brief Overview
The Stormont Vail Health Command Center is a team working together to ensure that patients receive the right care, in the right setting, at the right time. Team members that work in the Command Center are responsible for health system capacity management including patient access and throughput, staffing resources and communication to internal and external stakeholders. The Command Center Associate is responsible to assist in coordinating transfer into/out of the system, activating teams for critical/complex conditions, and expediting patient throughput. The position is accountable for a high volume of calls for internal and external stakeholders.

Education Qualifications
  • High School Diploma / GED High School Graduate or equivalent Required
  • Bachelor's Degree Preferred

What you will do
  • Assist House Supervisor / Patient Placement Coordinators with Capacity Management workflows.
  • Assist with the coordination of patient transfers; collecting patient information on condition and diagnosis, obtaining face-sheet, and collecting medical records/images.
  • Serve as a Liaison to provide real-time bed status and updates to referring facilities.
  • Communicate status updates with transferring facilities and update House Supervisors regarding clinical changes.
  • Coordinate transportation for lateral or higher of care transfers (from ED and inpatient departments).
  • Assist with Out-of-Network transfer workflows.
  • Facilitate Provider to Provider communications.
  • Assist House Supervisor with maintaining appropriate staffing assignments.
  • Expedite patient flow through communication/prioritization of resources (EVS, Facilities, etc).
  • Coordinate delivery of stat medical record request (for requesting facilities).
  • Continuously utilize system data and dashboards for real-time status updates.
