Qualifications

• Master's degree in a behavioral health field required.
• 2 years of experience providing evidence-based treatment for behavioral health diagnoses.
• Licensure required: LCSW, LPC or LMFT upon hire.

Responsibilities

• Provide high-quality care to patients as an independent health care provider.
• Collaborate with primary care staff and the team psychiatrist to deliver brief interventions for adult patients in Primary Care.
• Provide short, intensive follow-up.
• Use outcome-based models to track patient improvement.
• Participate in multidisciplinary care teams within the Collaborative Care workstream.
