Qualifications

• The successful NP must hold a Master’s Degree and be eligible for licensure as a PA in PA.
• Certification as a Nurse Practitioner and national certification eligibility are required or anticipated.
• Epic experience is preferred.

Responsibilities

• Perform comprehensive medical assessments.
• Provide medication management, evaluations, and daily patient rounding.
• Develop and implement treatment plans with psychiatrists, psychologists, nurses, therapists, and case managers.
• Document all care in the Epic electronic health record.
• Participate in treatment team meetings and coordinate care transitions.
• Communicate with families, caregivers, and community providers to support continuity of care.
