Job summary

Sevita seeks a Cognitive Retraining Specialist to provide facility-based individual and family therapy. You will assess ongoing needs for cognitive services in residential settings and work with a treatment team to develop and monitor IRPs.

This role requires license/certification as applicable and travel as needed. Qualifications include a Bachelor’s degree with CSW or teaching certification, a Master’s degree preferred, and at least one year of experience with developmental disabilities or

Qualifications

Bachelor’s degree plus a CSW or teaching certification.
Master’s degree in human services preferred.
Minimum of one year of experience with clients with development disabilities or mental health issues.

Responsibilities

Maintains a caseload of clients and provides direct client services.
Evaluates client needs and develops an IRP with a team.
Creates, coordinates and evaluates service plans with stakeholders.
Provides case management with attention to insurance needs.
Assesses referred clients and leads service-plan development with the treatment team.
Participates in team conferences and leads conferences.
Completes documentation and maintains clinical records and progress notes.
Travels as needed and adheres to regulatory standards.
