Job summary

Avera Health is seeking a Coder II to accurately assign outpatient ICD-10-CM, CPT and HCPCS codes across multiple facilities in Sioux Falls. You will review clinical documentation to ensure proper coding sequences and maintain quality and production goals with some guidance from senior staff.

Responsibilities include querying physicians, resolving documentation gaps, and liaising with HIM, billing and providers to support accurate coding and reporting.

Qualifications

  • AHIMA/AAPC certifications within 180 days of hire as listed.
  • Outpatient coding knowledge across ICD-10-CM, CPT, HCPCS.
  • Ability to review clinical documentation and extract accurate codes.

Responsibilities

  • Review patient charts to assign ICD-10-CM, CPT, and HCPCS codes across facilities.
  • Understand ICD-10-CM, CPT, HCPCS and update knowledge through study.
  • Determine APC/EAPC and outpatient coding for ER, imaging, labs, therapy, dialysis, etc.
  • May serve as subject matter expert for others at Avera.
  • Query clinicians to ensure full capture of the medical record.
  • Coordinate with HIM, billing, and providers; maintain quality metrics.

A Brief Overview

Responsible for the timely and accurate assignment of diagnostic and procedural codes for a variety of outpatient charts for multiple facilities within Avera Health.

Accurate abstracting, along with other reporting and editing functions is also a major responsibility.

The Coder II works independently to meet quality and production goals for the position with occasional guidance from other professional staff.

What you will do
  • Review all aspects of a patient's clinical documentation in order to identify the appropriate sequence of ICD-10-CM, CPT, and HCPCS diagnosis and procedure codes for assigned patient charts across Avera's facilities.
  • Understands ICD-10-CM, CPT and HCPC codes in depth, and be willing to update that knowledge through research or other educational opportunities.
  • The Coder II focuses on determining the appropriate APC/EAPC and/or medical coding for a variety of outpatient patient types including but not limited to, ER, ancillaries, imaging, lab, PT, OT, SP and/or dialysis in the outpatient service line.
  • Be willing on occasion to serve as a subject matter expert for other health professionals within Avera on these topics.
  • Query physicians and clinical documentation staff to ensure a full capture of the clinical record.
  • Assist with or lead communication between health information management, billing, and providers as needed.
  • Maintains personal quality and production statistics in accordance to service line standards for a Coder 2.

Required Education, License/Certification, or Work Experience:
  • Certified Coding Associate (CCA) - American Health Information Management Association (AHIMA) within 180 Days
  • Certified Coding Specialist - Physician-based (CCS-P) - American Health Information Management Association (AHIMA) within 180 Days
  • Certified Coding Specialist (CCS) - American Health Information Management Association (AHIMA) within 180 Days
  • Certified Outpatient Coder (COC) - American Academy of Professional Coders (AAPC) within 180 Days
  • Certified Professional Coder (CPC) - American Academy of Professional Coders (AAPC) within 180 Days
  • Registered Health Information Administrator (RHIA) - American Health Information Management Association (AHIMA) within 180 Days
  • Registered Health Information Tech (RHIT) - American Health Information Management Association (AHIMA) within 180 Days
Preferred Education, License/Certification, or Work Experience:
  • Associate's Health Information Administration or Health Information Technology 1-3 years coding experience
