Responsibilities

• Provides bedside clinical leadership for a defined patient cohort with rounds, assessment, chart review, and patient/family interaction.
• Performs real-time clinical surveillance and escalation for patient condition changes and safety risks.
• Coordinates interdisciplinary care through rounds and discharge planning, aligning stakeholders around patient goals.
• Implements evidence-based protocols across mobility, safety, and clinical care practices.
• Uses dashboards and data to drive quality improvements and education at unit level.
• Coaches RNs and nursing staff on practices, escalation criteria, and patient engagement.
