Job summary

AccessHealth is seeking a Certified Nurse Assistant (CNA) to provide dedicated home care services. The role involves direct patient care, personal hygiene, transfers, and ambulation within clients' homes across designated areas.

Travel is required, with emphasis on compassionate, high-quality care and adherence to the Plan of Care. This role does not imply an employment contract or guaranteed hours, and candidates must be physically capable of the job's demands.

Qualifications

• Must hold DFS certification.
• High school diploma or GED preferred.
• Strong communication abilities; read, write, and follow directions.
• Physical ability to lift up to 50 lbs and perform bending/standing tasks.
• Compassionate, patient-centered attitude.

Responsibilities Include

• Providing direct patient care, including personal hygiene, transfers, repositioning, and ambulation.
• Administering basic nursing care and prescribed exercises under supervision.
• Preparing meals considering client preferences and dietary needs.
• Offering companionship and engaging in activities tailored to individual interests.
• Performing light housekeeping duties and maintaining accurate records.
• Monitoring and reporting changes in the client's physical or psychosocial condition.
• Completing tasks via EVV app or time sheets, demonstrating dependability and punctuality.
• Supporting client independence within their abilities and respecting their privacy and rights.
• Adhering to the Plan of Care, always refraining from medication administration.

Qualifications And Skills

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills, ability to read, write, and follow directions.
