Maple Grove Wellness & Rehabilitation in Missouri is seeking a reliable CNA / Transportation Driver to join our team. This dual-role position safely transports residents to medical appointments and provides hands-on care as a CNA when transportation isn’t needed.

Qualifications

• High School Diploma or GED is required.
• CNA certification is required.
• Valid Class E driver’s license with a clean driving record is required.
• BLS/CPR certification is required.
• Experience in a skilled nursing facility or long-term care setting preferred.

Responsibilities

• Coordinate and assist with scheduling resident transportation for medical appointments.
• Safely transport residents to and from appointments using the facility vehicle.
• Assist residents during transport, including wheelchair and mobility support.
• Communicate with nursing staff regarding resident needs and appointment details.
• Maintain vehicle cleanliness, fuel levels, and report maintenance needs.
• Provide direct resident care per care plans and assist with ADLs.
• Support nursing staff with routine care tasks and monitor changes in resident condition.
