HCR Home Care in Plattsburgh, NY is seeking a Part Time Certified Nursing Assistant to participate in a transition program to become a Home Health Aide (HHA) for Clinton County. Earn while you learn through an NYS DOH approved course that leads to an HHA certificate and home care patient visits.

Requirements include an active CNA certificate, HS diploma or GED preferred, and successful completion of HCR's Reading and Math Test.

Earn while you learn! HCR is pleased to announce our ONE day CNA Transition Class to obtain a HHA Certificate. This NYS Dept of Health approved course will enable participants to obtain a recognized HHA Certificate and work for HCR seeing our home care patients. Work is readily available in Clinton County!

Position Summary: Home Health Aides provide personal care and supportive light housekeeping services to individuals in the community in the privacy of their home.

Essential Functions

• Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.
• Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
• Immediately report changes in client’s condition to supervising professional.
• Ensure the safety of the patient, patient’s family and/or the employee.
• Maintain HIPAA.
