Job summary

Lutheran Home Cottages in Clinton, NY is seeking a Certified Home Health Aide (HHA) to provide personal care to patients at home under physician orders and guidance from a Registered Nurse or Physical Therapist.

You will implement care plans, monitor health status, assist with daily living activities, report changes, document care, and maintain HIPAA confidentiality. HHA certification required; High School Diploma or GED accepted; experience with older adults preferred.

Responsibilities

• Implement the care plan established by the RN or therapist.
• Notify progress to the RN/Supervisor and adjust the plan as needed.
• Report changes in patient status promptly.
• Provide personal care and assistance with ADLs.
• Monitor vital signs and document observations.
• Assist with transfers, mobility, and elimination.
• Maintain patient safety and confidentiality per HIPAA.
• Document care and participate in in-service training.

Job Location: CLINTON, NY 13323, Salary Range: $18.65 - $18.65 Hourly, Job Shift: Any, TITLE: Certified Home Health Aide (HHA) Must be certified!!

Position Scope

Administers personal care and assistance with activities of daily living for patients in the home setting under physician’s orders, assignment and supervision provided by the Registered Nurse or Physical Therapist.

Responsibilities

• Implement the care plan established by the Registered Nurse, or other Professional.
• Notify the Registered Nurse/Supervising Nurse/Administrator of patient’s progress/lack of progress and the need to adjust the care plan.
• Promptly report changes in patient’s physical and/or mental status.
• Administer or assist patient with personal care, hygiene and grooming, including bath or shower, hair, skin, mouth, nail care, dressing and undressing
• Monitor and record vital signs.
• Assist patient with elimination using bathroom, commode, bedpan, or urinal.
• Assist patient with safe transfer and ambulation activities.
• Assist with urinary catheter irrigation and with insertion of rectal suppositories
• Assist patient with prescribed exercise after instruction by the Registered Nurse or Physical Therapist.
• Assist with household services that are essential to the patient’s home health care.
• Observe, report, and document patient status, care or service furnished, submitting activity records daily. Verbal reporting of pertinent observations of patients’ physical and psychosocial status to Registered Nurse, Therapist, or Supervising Nurse when immediate changes are needed.
• Document accurately and concisely all care and observations.
• Attend and participate in pertinent in-service education programs.
• Maintain strict confidentiality.
• Performs other duties as required.

HIPAA Compliance

Demonstrates knowledge of and adheres to Lutheran Home Cottages policies and HIPAA regulations governing the confidentiality and security of protected health information for both manual and electronic resident records, charts, information and other confidential information including (but not limited to) employee records, computer access codes and passwords.
