We are seeking a Hospice CNA/STNA to support our expanding Hospice services. This role complements our strong independent senior services, outpatient capabilities, and clinical facilities.

Shifts Available: PRN & Full-time

Responsibilities

• Provide personal care to patients
• Ensure care is delivered with respect and dignity, respecting patient rights
• Maintain accurate hospice care plans for each patient
• Observe, record, and report patient status, symptoms, and behaviors
• Assist with feeding as needed
• Communicate effectively with patients, families, and staff
• Attend required in-services and team meetings
• Participate in quality improvement activities
• Travel between Lebanon, Middletown, and Springboro to see patients

Qualifications

• Minimum age of 18
• High school diploma or GED; 9th grade reading/writing skills sufficient
• Valid Ohio CNA/STNA certification and active registry status; CPR certification required
• At least 1 year of direct patient care experience; hospice experience preferred
• Valid driver’s license and reliable, insured transportation
