Qualifications

• Minimum 1 year of experience working under an active CNA Certification.
• Minimum 1 year of experience working in skilled nursing.
• High School Diploma or equivalent required.
• Must have reliable transportation to assigned facilities.
• Must maintain consistent attendance and punctuality.
• Able to push, pull, and lift at least 50 pounds.
• Able to stand and walk with frequent bending, reaching, and lifting with proper body mechanics.
• Must be legally authorized to work in the United States.
• Willing to complete background and drug screening in accordance with applicable laws and regulations.

Responsibilities

• Utilize EMR Matrix to record patients’ medical information and vital signs.
• Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting.
• Provide safe transfers, repositioning, and mobility assistance using proper body mechanics.
• Obtain and document vital signs and other resident observations as assigned.
• Respond promptly to resident call lights and requests for assistance.
• Report changes in resident condition, behavior, or concerns to the nursing team.
• Assist residents with meals, hydration, and feeding when needed.
• Maintain a clean, safe, and respectful environment while following infection control practices.
• Support the nursing team in delivering quality resident-centered care.
• Communicate concerns or issues to onsite supervisor and agency.
• Other duties as assigned.
