SnapCare in Medford, Oregon is seeking a Certified Nursing Assistant to provide compassionate daily care for residents, assisting with activities of daily living and monitoring health, reporting changes to the supervising nurse. You will collaborate with the care team to maintain dignity and safety.

Applicants must hold an active Oregon CNA license, CPR/BLS certification, and complete onboarding. Prior skilled nursing or long-term care experience is preferred; strong communication and teamwork

Qualifications

• Active Oregon CNA certification in good standing.
• Current CPR/BLS certification as required by the facility.
• Previous skilled nursing or long-term care experience preferred.
• Ability to perform essential duties safely, with or without reasonable accommodation.
• Strong communication, teamwork, and time management skills.
• Completion of required onboarding, credentialing, and facility orientation before working.

Responsabilités

• Assist residents with bathing, dressing, grooming, toileting, and other activities of daily living.
• Support safe transfers, repositioning, and mobility using appropriate equipment.
• Assist with meals and hydration according to resident care plans.
• Obtain and document vital signs and other assigned observations.
• Promptly report changes in condition, safety concerns, and resident needs to the supervising nurse.
• Maintain accurate documentation and protect resident confidentiality.
• Follow infection prevention practices, facility policies, and individualized care plans.
• Promote resident dignity, comfort, independence, and safety.
• Arrive on time and communicate professionally with residents, families, and facility staff.
