Job summary

Neighborhood at Decatur, operated by Phoenix Senior Living, is seeking a Resident Care Associate (CNA) to join the team. The role provides hands-on care to residents in assisted living and memory care settings, following each resident’s PSP to ensure safety and comfort.

Responsibilities include updating service plans, meal and hydration support, daily room checks, activity facilitation, and prompt response to call bells, while adhering to regulatory standards and facility policies.

Qualifications

• High School education preferred and may be required by the state.
• Previous experience working with elders or disabled individuals, preferred.
• Must be at least 18 years of age to perform the personal care aspects of the RCA position.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
• Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times.

Responsibilities

• Participates in development of Personalized Service Plans and updates; attends service plan review meetings with families as requested.
• Follows hydration schedule established for residents and maintained during the shift.
• Completes resident meal attendance sheets and ensures those not dining are served room service or accounted for.
• Knows and complies with all resident rights.
• Initiates, assigns, and assists with activities, as appropriate.
• Check all assigned residents daily for cleanliness, clothing, continence, and changes in condition.
• Uses proper sanitary procedures and universal precautions.
• Checks resident rooms daily for cleanliness; ensures high risk rooms are double checked.
• Assists with cleaning of laundry rooms and common areas; coordinates with housekeeping as needed.
• Completes all ADLs for assigned residents per PSP.
• Responds to security system and resident call bells promptly; resets bells as needed.
