Job summary

Interim HealthCare Inc. is seeking a compassionate Hospice Certified Nursing Assistant (CNA) to join our hospice care team in the Northwest Houston area.

The CNA will provide high-quality, personalized end-of-life care to patients in their homes, ensuring comfort and dignity while supporting the patient and family. The role requires a Texas CNA license, 1+ year of experience preferred, and the ability to travel to patient homes.

Qualifications

• Licensed CNA in Texas is required.
• Hospice or home health experience is preferred.
• Effective written and verbal communication is essential.

Responsibilities

• Provide hands-on assistance with activities of daily living (ADLs) including bathing, dressing, grooming, and monitoring vital signs.
• Document changes in patient condition and report to the RN or hospice team.
• Assist in administering medications per prescribed plans and report adverse reactions.
• Maintain a clean, safe home environment and implement pain management as outlined in the care plan.
• Maintain detailed patient records and ensure compliance with hospice documentation requirements.
• Collaborate with the interdisciplinary team to deliver holistic care and participate in care planning discussions.

Responsibilities

• Patient Care:
  • Provide hands‑on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.
  • Document any changes in the patient's condition and report concerns to the registered nurse or hospice team.
• Medication Administration:
  • Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.
  • Maintain accurate records of medication administration.
• Comfort Care:
  • Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well‑being.
  • Implement appropriate pain management techniques as outlined in the patient's care plan.
• Documentation:
  • Maintain detailed patient records, documenting care provided, observations, and any changes in the patient's condition.
  • Ensure compliance with all documentation and reporting requirements as per hospice regulations.
• Collaboration:
  • Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.
  • Participate actively in team meetings and contribute to care planning discussions.

Qualifications
