Job summary

Bristol Hospice is seeking a Certified Home Health Aide to join our Interdisciplinary Group Team on a PRN basis in the Twin Cities area. You will perform personal care, assist with daily living activities, and provide compassionate, respectful care under RN supervision in patients' homes.

Ideal candidates have CNA certification, CPR, and a willingness to work autonomously with strong communication and organizational skills. A valid driver’s license and background check are required.

Qualifications

• Must have CNA certification and CPR certification.
• Ability to follow written care plans and communicate effectively with patients and families.
• Strong clinical judgment to assess patient needs and report changes to the Case Manager.

Responsibilities

• Provide personal care and comfort to patients in their homes.
• Assist with activities of daily living and basic treatments under RN supervision.
• Maintain safety and proper use of equipment; document care provided.
• Communicate patient needs and changes to the Case Manager promptly.
• Support family caregivers and provide respite as needed.
