Certified Nursing Assistant (CNA) – PM Shift

Location: San Joaquin Nursing and Rehabilitation, Bakersfield, CA

Position: Full‑time, 2:30 PM–10:30 PM (7.5‑hour shift) – pay $18.00 / hr

San Joaquin Nursing and Rehabilitation is seeking a full‑time CNA for our PM shift to be part of a compassionate, skilled team in a dynamic and supportive environment. As a premier 99‑bed skilled nursing facility in Bakersfield—located across from the city’s largest hospital—we provide 24‑hour personal and clinical care to a diverse resident population.

We are proud to be the first facility in Kern County to implement Non‑Invasive Ventilator programs, and we continue to lead with innovation, integrity, and compassion.

General Purpose

Provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan and as directed by supervisors.

Essential Duties
  • Use wristband or photo card file to identify residents before administering treatments, serving meals, etc.
  • Use only authorized abbreviations established by the facility when recording information.
  • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
  • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
  • Report all accidents and incidents observed on the shift.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure.
  • Report any known or suspected unauthorized attempt to access the facility’s information system.
  • Perform only nursing care procedures that you have been trained to do.
  • Ensure the resident’s room is ready for receiving the resident (bed made, name tags, admission kit).
  • Greet residents and escort them to their room.
  • Introduce resident to his/her roommate and other residents and personnel as appropriate.
  • Make resident comfortable.
  • Inventory and mark the resident’s personal possessions as instructed.
  • Store resident’s clothing.
  • Assist residents with packing their personal possessions when being transferred or discharged.
  • Transport residents to new rooms or receiving area.
  • Assist with loading/unloading residents to/from vehicles as necessary.
  • Create and maintain an atmosphere of warmth and positive emphasis throughout the unit and shift.
  • Meet with shift nursing personnel regularly to assist in identifying and correcting problem areas.
  • Report all complaints and grievances made by the resident.
  • Participate in and receive the nursing report as instructed.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds as instructed and put extra covers on beds as requested.
  • Measure and record temperatures, pulses, and respirations (TPRs) and weigh residents as instructed.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure their personal care needs are met.
  • Assist residents with identifying food arrangements (food on tray, location, temperature).
  • Record the resident’s food/fluid intake and report changes in eating habits to the Nurse Supervisor/Charge Nurse.
  • Keep residents’ water pitchers clean, filled with fresh water each shift, within easy reach.
  • Perform after‑meal care (remove trays, clean hands, face, clothing).
  • Perform all assigned tasks in accordance with our established policies and procedures and as instructed by supervisors.
  • Follow work assignments and schedules in completing tasks.
  • Cooperate with inter‑departmental personnel to ensure nursing services can be adequately maintained.
  • Notify the facility when you will be late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policies.
  • Assist residents with daily functions (dental and mouth care, bath functions, combing hair, dressing and undressing as necessary).
  • Keep residents dry (change gown, clothing and linens when wet or soiled).
  • Assist residents in preparing for medical tests (lab work, x‑ray, therapy, dental).
  • Assist with lifting, turning, moving, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing the resident for a physical examination.
  • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
  • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers.
  • Report injuries of an unknown source, including skin tears.
  • Observe disoriented and comatose residents. Record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
  • Use only equipment you have been trained to use.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality of all resident care information including protected health information.
  • Report known or suspected incidents of unauthorized disclosure of such information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor the resident’s refusal of treatment request. Report such requests to your supervisor.
Supervisory Requirements

This position has no supervisor responsibilities.

Qualification

Education and Experience

  • Must possess a minimum of a 10th grade education.

Language Skills

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.

Mathematical Skills

  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations

  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
  • CPR Preferred.

Frequent use of all office related equipment to include copier/scanner/fax, telephone, and calculator. May be necessary to assist in the evacuation of residents during emergency situations.
