Qualifications

• NYS CNA credentials required.
• High school diploma or equivalent with reading/writing at 12th grade level or higher.

Responsibilities

• Assist residents per care plan and shift priorities.
• Report to licensed nurse with pertinent information.
• Provide safe, clean resident environment and document ADLs.
