Job Summary:

Assists in performing a variety of nursing care services for patients in a long-term care setting.

Job Responsibilities and Requirements:
  1. Primary Responsibilities:
    • Receive patient care assignments from the registered nurse (RN) in charge and initiate delegated tasks based on demonstrated competence.
    • Collect and document data for admission assessments and reassessments, providing data to RN and/or LPN for analysis.
    • Contribute to safety and quality care through charting and communication.
    • Inform licensed nurse of any changes in patient condition or needs.
    • Respond to patients' calls for help, take vital signs, monitor behavior and physical condition, and assist with personal care activities.
    • Prepare patients for discharge and accompany them as needed.
    • Stock and maintain supplies and equipment efficiently.
    • Assist with clerical duties such as answering phones and organizing medical records.
    • Orient new patients and significant others to the nursing unit.
    • Operate mechanical and manual equipment safely and recognize mechanical issues.
    • Work in a constant state of alertness and safety.
    • Perform other duties as assigned.
  2. Education: Completion of an approved nurse aide training program and passing the state exam.
  3. Experience: No experience required.
  4. Physical Requirements: Constant standing and walking, frequent use of hearing and speech, lifting/carrying objects 0-25 lbs, and other physical activities as detailed.
  5. Required Professional License and Certifications: CNA license valid in Illinois, Missouri, Oklahoma, or Wisconsin, depending on location.
