Job summary

Trinity Health PACE Alexandria is seeking a compassionate Certified Nursing Assistant (CNA) to join a multidisciplinary team serving older adults in Central Louisiana. The CNA will assist with ADLs, transfers, nutrition, and observational care across the PACE center, home, and community settings.

You will participate in an interdisciplinary team, document care, and support participant independence while upholding dignity and safety. A valid Louisiana CNA license and CPR/BLS are required.

Qualifications

• Current Louisiana Certified Nursing Assistant (CNA) certification in good standing.
• High school diploma or equivalent.
• Current CPR/BLS certification or ability to obtain upon hire.
• Valid driver's license and reliable transportation.
• Ability to lift, transfer, and assist participants safely.

Responsibilities

• Assist participants with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, eating, and mobility.
• Assist with transfers, ambulation, and positioning using safe techniques and equipment.
• Provide restorative and supportive care to promote independence and dignity.
• Assist with participant meals, hydration, and nutritional support as directed.
• Maintain participant comfort, privacy, and confidentiality.
• Observe changes in physical, cognitive, emotional, or behavioral status and report them promptly.
• Document services provided per program standards and regulatory requirements.
• Support participant arrival, departure, and transportation coordination.
• Engage participants in recreational, social, and therapeutic activities.
• Maintain a clean, safe, and organized care environment.
• Assist with setup and cleanup of activities and meals.
• Provide home and community support during visits and outings.
• Observe and report environmental or safety concerns in participant homes.
