The Certified Nurse Assistant (CNA) or Home Health Aide provides personal care assistance and home support to maintain a clean, safe environment for elderly or convalescing patients, enabling them to remain in their own homes. CNAs and Home Health Aides work in patient homes and facilities, interacting with residents, family members, and staff under all conditions. They communicate regularly with medical staff, nursing personnel, and other units. CNAs and Home Health Aides operate under the supervision of a Registered Nurse and the Scheduling Coordinator.

Location: East Falls /Philadelphia PA

Schedule:

• 9 am - 5 pm Sunday through Friday
• 12-6 pm Saturday
• Part-time every other weekend

Primary Job Responsibilities

• Providing frail, ill, or immobile clients with assistance, company, and comfort
• Executing the patient care plan and communicating effectively with RN Case Manager
• Monitoring clients' physical, mental, and emotional conditions and reporting changes
• Performing personal hygiene care
• Performing light housekeeping in the patient's living area
• Following nutritional guidelines and assisting with meals
• Supporting clients with incontinence care and bathroom visits
• Running errands for the patient, such as grocery shopping
• Assisting clients with mobility from beds to chairs or wheelchairs
• With prior approval, accompanying clients on trips like medical appointments
• Maintaining a clean, safe environment
• Documenting care using the agency's Electronic Visit Verification system
• Adhering to schedule and communicating scheduling needs
• With proper training and prior approval, administering prescribed medicines and treatments

Requirements

• Effective reading, writing, and communication skills
• Ability to understand written and verbal instructions
• Valid Pennsylvania CNA license
• Successful completion of a knowledge exam
• Strong organizational skills
• Compassionate attitude towards patient care
• Ability to document accurately and thoroughly
• One year of home health experience preferred
• Proof of COVID-19 and Influenza vaccinations or approved exemptions
• Valid driver's license and auto insurance if in a driver role
