Job summary

Vicar's Landing Oakbridge Memory Care in Ponte Vedra Beach, FL is seeking a compassionate CNA to provide daily, resident-centered care in a Montessori-inspired environment. You’ll work closely with residents to support independence, dignity, and meaningful engagement while following individualized care plans.

You’ll perform ADLs, assist with meals, document care in electronic medical records, and report changes promptly.

Qualifications

• Active CNA certification.
• Current BLS/CPR certification.
• Ability to provide compassionate, resident-centered care.
• Strong communication skills and ability to work as part of a team.
• Ability to follow care plans and document accurately.

Responsibilities

• Provide daily hands-on care and support to residents in accordance with individualized care plans.
• Assist with activities of daily living such as personal care, mobility, and comfort needs.
• Support residents during meals and assist with dining as needed.
• Participate in resident admissions, transfers, and discharges as directed.
• Observe residents closely and report any changes to nursing staff promptly.
• Document care and activities accurately in electronic medical records.
• Maintain a safe, clean, and infection-control compliant environment.
• Assist with meaningful activities and encourage resident engagement and independence.
• Treat residents and their belongings with dignity, respect, and compassion at all times.

What You'll Do
