Job summary

Rainbow Community Care is seeking a dedicated Full Time CNA to work at the Inpatient Center in Johnson Creek. The role is 0.9 FTE (36 hours per week) with primarily day shifts.

You will provide compassionate care, assist with activities of daily living and support patients and families on their care journey. Qualifications include Wisconsin Nursing Assistant Certification and at least some CNA experience; valid driver’s license and insurance are required for travel between locations as needed.

Qualifications

• CNA certification is required.
• 6 months CNA experience preferred.
• Must be able to drive to multiple locations as needed.

Responsibilities

• Assists with ambulation, transferring and changing patient positions.
• Assists with personal grooming, toileting, bathing, skin and nail care, dressing and walking.
• Helps with meals and assists patients with eating and drinking.
• Provides emotional and psychological support to patients and families.
• Keeps a clean, safe environment and performs light housekeeping.
• Follows the care plan and documents provided care.
• Reports changes in patient condition per Rainbow policies.

Purpose of the Position

Key Responsibilities

• Assists with ambulation, transferring, and changing position of patients.
• Assists the patient with personal grooming, toileting, incontinence care, bathing, skin care, nail care, dressing, transferring and walking.
• Assists with preparing meals and assists patients with eating and drinking.
• Gives emotional and psychological support to the patient/family.
• Maintains a clean, safe and comfortable environment. Performs light housekeeping duties as assigned.
• Reads, understands, and follows the plan of care.
• Maintains up-to date patient records and documents cares provided to the patient.
• Reports changes in patient condition according to Rainbow policies.

Qualifications

Professional Qualifications

• Valid Wisconsin driver’s license and proof of current car insurance are required upon hire and must be maintained throughout employment.
• Must be able to drive to and from other office/meeting locations as necessary.
• Current Wisconsin Nursing Assistant Certification is required.
• Minimum of 6 months CNA experience preferred.
• Hospice experience preferred.
