Job summary

Rocky Mountain Care Hospice in Murray, Utah, is seeking a compassionate Hospice CNA to provide one-on-one care in patients' homes or care settings, following the individualized plan of care.

You will assist with ADLs, mobility, and comfort, observe changes, document accurately, and report to the hospice nurse. Join a team dedicated to dignity, respect, and exceptional care at a time when it matters most.

Qualifications

• Current CNA certification in good standing.
• CPR/BLS certification as required.
• Valid driver's license with reliable transportation and auto insurance.

Responsibilities

• Assist patients with bathing, grooming, dressing, toileting, and other activities of daily living (ADLs).
• Help with positioning, transfers, mobility, and comfort needs.
• Provide compassionate, one-on-one care in patients' homes or care settings.
• Observe and report changes in patient condition to the hospice nurse.
• Complete required documentation accurately and timely.
• Support patients and families with kindness, dignity, and respect.
• Follow the patient's individualized hospice plan of care.
