Job summary

Interim HealthCare is hiring Hospice/Home Health CNAs to provide one-on-one care for patients in Fort Wayne, IN and surrounding areas. You will care for seniors in their homes, assist with daily activities, monitor vitals, and collaborate with a team of hospice nurses to follow each patient’s care plan.

We offer a competitive pay of $20.00–$23.00 per hour, mileage reimbursement, flexible schedules, and no nights or weekends, with 1:1 CNA-to-patient ratios.

Qualifications

• Must have a valid Indiana CNA certification.
• High school diploma or equivalent required.
• CPR certification required.
• Valid driver’s license and transportation.

Responsibilities

• Care for seniors in patients' homes and in hospice settings.
• Assist with bathing, toileting, dressing, grooming, hygiene, and nutritional intake.
• Support with ambulation, transfers, and range-of-motion exercises.
• Monitor vitals, intake/output, blood glucose, and document changes.
• Notify supervisor of changes in care or equipment needs and coordinate with the care team.
