Job summary

Amedisys is seeking a CNA Hospice Aide to join our team in Lakeland, FL. This Fri-Sun 12-hour shift role focuses on compassionate home-based hospice care, with responsibilities spanning personal care, safety-focused support, meal preparation, and daily living assistance.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant per state requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to personal care tasks such as ambulation, transfers, ROM and positioning, and homemaking.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes, reports, and documents changes in patient’s status to the nurse.
• Follows written nurse instructions for care as per plan of care.
• Assists with prescribed exercises as instructed by a nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
