Role Responsibilities
  • Report directly to the Administrator, Executive Director or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
  • Help the patient maintain good personal hygiene.
  • Assist in maintaining a safe and healthy environment.
  • Prepare and serve light meals.
  • Assist in feeding patients when needed.
  • Assist the patient in ambulation.
  • Assist with the safe use of mobility aids like walkers or wheelchairs as taught and supervised by the nurse.
  • Assist with light housekeeping tasks such as trash removal and linen changes.
  • Assist with treatments as ordered in the aide plan of care and in accordance with state regulations.
  • Promote patient’s mental alertness through engagement in activities of interest.
  • Provide basic emotional and psychological support to patients and households.
  • Report any changes in patient condition or home situation to the RN and team.
  • Demonstrate basic care to primary caregivers.
  • Practice infection control and standard precautions.
  • Ensure accurate documentation of visits and activities.
  • Attend staff meetings and participate in quality improvement activities.
  • Complete documentation promptly as per orientation guidelines.
Qualifications
  • Compassionate, kind, and empathetic nature.
  • Comfortable providing care in a patient's home environment.
  • Ability to remain flexible and composed under pressure.
  • Certified Nursing Assistant (CNA) license.
  • Six months of patient care experience in relevant settings preferred.
  • Additional experience in hospital or emergency settings advantageous.
  • Current driver’s license and automobile insurance.
  • Current CPR certification.
