Responsibilities

  • Provides personal care services to patients.
  • Applies safety principles and proper body mechanics to perform specific techniques of personal and supportive care such as ambulation, transferring, assisting with motion and positioning, and household chores.
  • Plans and prepares nutritious meals, assists with feeding, as assigned.
  • Performs homemaking and environmental services as assigned.
  • Observes, reports, and documents any changes in patient's status. If unable to perform a task, report to the primary nurse.
  • Follows specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Instructions are to be received prior to service delivery.
  • Assists patients with prescribed exercises as instructed by nurse or therapist.
  • Performs and records simple urine tests for sugar, acetone, and albumin.
  • Other duties as assigned.

Qualifications

  • High school diploma or equivalent.
  • Current state certification (or registration) as a nurse assistant as per state requirements.
  • CN A experience within the last two years. If more than two years, complete another training and competency program before providing services.
  • Must pass the competency program of the care center.
  • Current CPR certification.
