Job summary

Transitions Hospice is seeking a compassionate Companion Aide CNA for the overnight shift covering Effingham, Mattoon and surrounding areas. You will provide continuous bedside presence, monitor safety, assist with personal care, and support families through the night with calm, professional care.

Requirements include CNA certification, CPR, and overnight care experience. You’ll work Mon-Fri 8 hour nights, with a focus on dignity, comfort, and patient safety in end-of-life care.

Qualifications

• Current CNA certification meeting state training requirements.
• Current CPR certification.
• 1+ year of hospice, home health, or related healthcare experience preferred
• Prior experience in overnight or night shift patient care preferred
• Ability to work independently and remain alert and attentive throughout overnight hours
• Strong observational skills with the ability to recognize and report changes in patient condition
• Genuine compassion for patients and families navigating end-of-life care

Responsibilities

• Providing continuous bedside presence and patient supervision overnight to ensure safety and comfort.
• Monitor patient condition, behavior, and environment; report changes to the on-call nurse.
• Prevent falls, elopement, or self-harm through attentive observation within CNA scope.
• Respond promptly to patient requests and maintain a calm, restful overnight environment.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
• Provide skin care and observe to prevent tissue breakdown during rest.
• Support normal range of motion and repositioning for comfort and skin integrity.

What You'll Do

As a Companion Aide CNA on the night shift, you'll be a steady, compassionate presence at the bedside — monitoring patient safety, providing personal care, and giving families the peace of mind that someone is watching over their loved one through the night. Your shift includes:

Overnight Patient Care & Safety

• Providing continuous bedside presence and patient supervision throughout the overnight shift to ensure safety and comfort
• Monitoring patient condition, behavior, and environment — and promptly reporting any significant changes to the on-call nurse
• Preventing falls, elopement, or self-harm through attentive observation and appropriate intervention within CNA scope of practice
• Responding promptly to patient requests and ensuring a calm, restful overnight environment
• Assisting with personal care including repositioning, toileting, bathing, hygiene, and dressing as assigned
• Providing proper skin care and observation to prevent tissue breakdown during prolonged periods of rest
• Supporting normal range of motion and repositioning to maintain patient comfort and skin integrity

Observation & Documentation

• Observing and documenting patient condition, sleep patterns, behaviors, and notable changes throughout the shift
• Taking and recording vital signs — temperature, pulse, respirations, blood pressure, and weights — when ordered and within demonstrated competency
• Reporting significant changes in patient condition to the RN or on-call supervisor promptly and without delay
• Adhering to the organization's documentation standards and professional conduct expectations

Family Support & Communication

• Providing families and caregivers with reassurance and overnight respite support
• Maintaining respectful, ethical communication with patients and families, including strict confidentiality
• Demonstrating comfort and compassion in providing specialized care to terminally ill patients and their families

Safety & Equipment

• Using patient equipment safely and appropriately — including foot stools, side rails, and oxygen
• Assisting with ambulation, exercise, and range of motion as directed by the hospice nurse or therapist
• Participating in quality assessment and performance improvement activities
