Responsibilities

• Provides competent nursing care to home health patients according to state and industry standards.
• Obtains vital signs of patients as assigned and monitors changes in condition.
• Communicates and documents tasks in a timely and accurate manner.
• Maintains a safe, clean and comfortable environment for the patient.
• Assists with personal hygiene, bathing, grooming and activities of daily living.
• Assists with meal preparation and related tasks.
• Assists with patient transfer, ambulation and exercise as prescribed.
• Records visits accurately and consistently.
