Qualifications

• Active CNA certification and state-issued credential

• English proficiency required; bilingual a plus

Responsibilities

• Provide non-medical in-home care and support with activities of daily living
• Monitor health and safety and document services accurately
• Assist with mobility, meals, transportation, and personal care
• Support participation in community activities and social engagement
• Report changes in condition to care team promptly
