As part of our interdisciplinary Hospice team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).
As a Home Health Aide in Hospice, you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. These activities are performed under the supervision of a Registered Nurse or Therapist, in accordance with physicians' orders.
- Support patient care by reviewing the care plan in the home prior to providing care. Notify the Case Manager of issues or concerns regarding the care plan and when cares need revision.
- Assist with daily living activities under the supervision of the Case Manager, such as bathing, dressing, transfers, range of motion exercises, toileting, etc.
- Explain to patients the care being provided and involve them in cares.
- Complete mobile care tasks upon arrival and departure from patients' homes per agency policy. Mobile care includes tasks in accordance with the care plan. Also, complete mileage logs accurately and timely.
- Work cooperatively with patients, care providers, family members, and agency staff by listening, responding to needs, engaging in communication, and reporting pertinent information to the Case Manager promptly.
- Perform home maintenance tasks as per the plan of care, such as house cleaning, washing dishes, laundry, meal preparation, and transportation.
