Role: CNA - Home Health Aide

Organization: PACE of West Baltimore

Location: 3201-A Tioga Parkway, Baltimore, MD 21215

Status: Full-time

Mission: To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives.

Job Summary: Under the supervision of the Home Care Coordinator, the CNA - Home Health Aide is responsible for providing and assisting participants with activities of daily living and other duties within their homes. Monitors health status, ensures a safe environment, promotes professional relationships, and adheres to organizational policies and standards.

Essential Duties and Responsibilities:

  1. Assist participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, nail care, and toileting.
  2. Follow scheduled assignments for daily and weekly tasks.
  3. Participate in care planning with interdisciplinary teams.
  4. Rotate into day center services, clinic duties, or community escort services as deemed necessary by management.
  5. Participate in Quality Improvement programs.
  6. Handle infectious specimens with proper precautions and follow universal safety practices.
  7. Support organizational problem-solving efforts.
  8. Attend continuing education and staff meetings.
  9. Maintain professional certifications and affiliations.
  10. Ensure compliance with infection control, PPE, blood-borne pathogens, hazardous materials, and fire safety standards.
  11. Keep detailed records of services, assessments, and participant wishes.
  12. Prepare and submit reports timely.
  13. Maintain confidentiality of participant information.
  14. Perform additional duties as assigned by management.

Qualifications:

  • At least one year of experience working with frail or elderly populations preferred.
  • Experience in long-term care or community-based geriatric programs preferred.
  • CNA License in Maryland required.
