Job summary

Complete Home Care seeks a compassionate Home Health Aide/Certified Nursing Assistant to provide in-home care across Florida. You will work with a dedicated team to deliver essential health services and support for seniors in their homes.

Requirements include 1 year of HHA/CNA experience, a High School Diploma or equivalent, an HHA certificate, CPR, a valid driver's license, reliable transportation, and the ability to lift up to 50 lbs. Full-time shifts and a positive team culture are offered.

Qualifications

Responsibilities

• Assist clients in their homes with personal healthcare tasks as outlined in the Plan of Care.

• Provide care per the Home Health Aide Plan and document care provided accurately.
• Contribute to continuity of care and coordinate with the care team in case conferences/meetings.
• Apply infection control and universal precautions to protect staff and patients.
• Participate in in-service training and maintain competency as assigned by supervisor.
• Meet ongoing continuing education requirements mandated by the agency and licensing board.

General Summary: Assists clients, in their place of residence, with personal healthcare and health maintenance activities as assigned. Includes, but is not limited to, transferring, health maintenance activities, feeding, mobility, and exercises.

Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.

Essential Functions:

1. Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).
2. Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
3. Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.
4. Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
5. Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
6. Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
7. Meet mandatory continuing education requirements of the Agency/licensing board.

Requirements

Experience:

• (1) year experience as a home health aide or certified nursing assistant in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Skills:

• Ability to establish and maintain effective working relationships with all segments of the staff, and the Agency leadership team.
• Shall be able to read, write and comprehend English.

Education:

• High School Graduate or equivalent

Licensure/Certification:

• Home Health Aide Certificate or other similar certification held in good standing. Certification requirements may vary by state and/or participating program.
• A formal certificate of completion for an HHA/CNA training program that meets or exceeds standards established by Health and Human Services. Completion of all in-service hours per year. Ongoing requirements may vary by state and/or participating program.
• Current CPR certification.
• Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
• Reliable transportation and auto liability insurance.
• On-going employment as a HHA always requires maintaining a valid HHA certificate in the state where the caregiver practices in and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.

Physical Requirements:

• Prolonged standing/walking is required.
• The ability to lift/transfer up to 50 lbs.
• Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
• Meet patient/family individualized psychosocial needs.
• Requires hand-eye coordination and manual dexterity.

Environmental/Working Conditions:

• Works in patients’ homes in various conditions.
• Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
• Some exposure to unpleasant weather; PRN emergency call.
