Job summary

Elk Valley, part of the LHC Group within the Optum family, is hiring a Per Diem Home Health Aide to assist clients at home.

You will provide personal care, help with daily living activities, monitor health, document notes, and ensure a safe environment while maintaining confidentiality.

Qualifications

• Required TN training with 16 clinical hours within first 3 months.
• Current driver's license with vehicle insurance and reliable transportation.
• CPR certification preferred or ability to obtain within 90 days.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Assist with household tasks, meal prep, and accompany clients to appointments or errands.
• Monitor client condition, vital signs, and incidents; maintain confidentiality and use EVV.
• Create a safe home environment and operate equipment as needed; respond to client needs promptly.
• Work flexibly in diverse home situations and respect all clients equally.

As the Per Diem Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminder
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification:

• Current driver's license, vehicle insurance, and reliable transportation or access to public transit

State-Specific Requirements

• TN: 75 hours of training including 16 clinical hours within first 3 months; qualified for basic services

Preferred Qualifications:

• Current CPR certification or ability to complete within 90 days of hire
• 6+ months of home care experience
• Ability to work flexible hours and independently

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $13.00 to $20.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
