Cantex Continuing Care Network is seeking a skilled Certified Nursing Assistant (CNA) for full‑time or PRN roles at Beacon Hill, Denison, TX. You will deliver direct patient care, assist with transfers, and support individualized care plans to improve outcomes.

Qualifications

• Active CNA certification required.
• Ability to lift/move and transfer patients safely.
• Desire to read, write and speak English clearly.

Responsibilities

• Deliver direct patient care including bathing, dressing and feeding.
• Assist with transfers, ambulation, and repositioning using safe lifting.
• Monitor and report changes in patient condition to the Charge Nurse.
• Document patient care and daily activities accurately in records.
• Support and follow individualized care plans to improve outcomes.
• Maintain a clean, safe, infection-controlled environment.
• Ensure equipment is clean, charged, and functioning properly.
• Participate in shift handoffs and team communication.
• Follow state, federal and facility policies and procedures.
• Promote patient dignity, independence and safety at all times.
