Job summary

Bend Transitional Care in Bend, Oregon seeks a compassionate Full-Time CNA I to join our 60-bed skilled nursing and rehabilitation team. You will assist with daily living activities, mobility, and monitoring residents’ health as you support their recovery and transition home.

Qualifications

• High school diploma or equivalent.
• Valid Oregon CNA certification.
• Valid CPR certification.

Responsibilities

• Providing assistance with activities of daily living, including bathing, dressing, grooming, and personal hygiene
• Assisting residents with mobility, transfers, and range-of-motion exercises
• Taking and documenting vital signs, food and fluid intake, and output as directed
• Assisting residents during meals and supporting their nutritional needs
• Observing residents and promptly communicating changes in condition to the nursing team
• Maintaining accurate and timely documentation
• Building positive, respectful relationships with residents, families, and team members
• Working collaboratively with the nursing team to provide exceptional resident care
