Job summary

Qualifications

• Excellent communication and interpersonal skills.

• Ability to safely assist residents with mobility and personal care.
• Accurately document resident information and report changes in condition.

Responsibilities

• Provide resident-centered care promoting comfort and dignity.
• Assist residents with ADLs including bathing, dressing, grooming, toileting, transferring, walking.
• Answer resident call lights promptly and provide assistance.
• Safely turn and reposition residents as needed.
• Measure and record vital signs (BP, pulse, temp, respirations).
• Monitor and document food/fluid intake and outputs.
• Observe and report changes in condition to the nursing team.
• Assist with meals following dietary restrictions and preferences.
• Remind residents to take medications and supplements as directed.
• Communicate with nurses, physicians, and caregivers.
• Maintain documentation per facility policies.
• Follow infection prevention and safety procedures.
• Stay current with CNA certification requirements.
