Responsibilities

• Provide personal care to residents.
• Assist residents with activities of daily living.
• Notify charge nurse of resident status at shift start and end.
• Contribute to resident care plans with observations.
• Assist in implementing care plans per policy.
• Observe and document residents' conditions.
• Ensure safe lifting and transfers per policy.
• Maintain clean, hazard-free environment for residents and staff.
• Perform incidental housekeeping as needed.
• Other duties as assigned.
