Job summary

Lightways Hospice, formerly Joliet Area Community Hospice, provides hospice and serious illness care in the northwest Illinois region. The Field CNA role supports patients in their homes, nursing facilities, or assisted living facilities under RN supervision, typically Monday–Friday 8AM to 4:30 PM.

Responsibilities include compassionate patient care, mobility assistance, and reporting changes to the RN while ensuring safety and comfort in patients' homes.

Qualifications

• Current CNA certification required.
• High school diploma or GED is required.
• Must have a valid driver’s license and a dependable, insured vehicle.

• Requires reliable personal transportation and willingness to travel to patient homes throughout North Cook and Lake Counties

About Lightways Hospice

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

Field CNA - Northern Cook and Lake County

The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The field CNA is responsible for observing the patient, reporting these observations and documenting observations and care performed. This role is Monday- Friday 8AM to 4:30 PM.

Key Responsibilities:

• Requires reliable personal transportation and willingness to travel to patient homes throughout North Cook and Lake Counties (including, but not limited to, communities such as Evanston, Glencoe, Arlington Heights, Schaumburg, and Deer Park)

Education/Experience:

• High school diploma or GED.


• Meets the training requirements of Illinois.


• Current CNA certification.


• Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.
