Qualifications

• Certified Nursing Assistant Certificate required.
• High school diploma or GED required.
• Ability to write routine reports.

Responsibilities

• Report to shift on time and spend 95% of hours in direct contact with individuals.
• Assist residents under supervision to ensure safety and comfort.
• Perform daily tasks and document care activities in clinical notes.
• Assist with medical needs: appointments, hospitalizations, transfers.
• Coordinate scheduling for OT/PT/ST as needed.
• Maintain resident dignity and advocate for rights per policy.
