Qualifications

• Must be a CNA and provide hands-on resident care.

• Provide compassionate, resident-centered care and documentation.

Responsibilities

• Provide hands-on care to residents, including activities of daily living, mobility, and incontinence care.
• Monitor and record vital signs, report changes to nursing staff, and update resident charts.
• Develop and maintain positive relationships with residents, families, and colleagues.
• Participate in interdisciplinary care planning and coordination.
• Maintain a clean, organized work environment with strict infection control.
• Collaborate with care staff to ensure smooth transitions and clear communication.
