Job summary

Skyline Healthcare Center-San Jose is seeking a Certified Nursing Assistant to provide compassionate patient and resident care on site. You will assist with daily living activities, monitor vital signs, and participate in care planning with the nursing team.

The ideal candidate holds a state-approved CNA certification (or equivalent) and a high school diploma. You will document care accurately, follow infection control procedures, and communicate changes to the supervisor to ensure timely

Qualifications

• Minimum high school diploma or equivalent or related experience.
• State-approved Nursing Assistant Certification or equivalent.
• Successful completion of CNA certification meeting state requirements.

Responsibilities

• Assist with activities of daily living (ADLs) and mobility.
• Record vital signs and weight as assigned.
• Document care accurately per policy and regulations.
• Provide non-resident services per infection control procedures.
• Make routine rounds and communicate changes to supervisor.
• Complete forms in accordance with state/federal regulations.
