Job summary

Hospice Austin is seeking a CNA for Christopher House in Austin, TX. This role supports patients under the direction of a Charge Nurse or Team Leader, performing essential personal care and routine tasks aligned with the patient care plan.

Responsibilities include hygiene, transfers, vital signs, feeding support, and documentation. Requires active Texas CNA certification and CPR certification; hospice/home health experience preferred. Shift is 11am–11pm, .9 FTE, 36 hours per week.

Qualifications

• Current, active Texas nurse aide certification (CNA).
• Listed in good standing on the Texas nurse aide/misconduct registry with no findings of abuse/neglect or misappropriation, verified by DHS.

Responsibilities

• Assist with personal hygiene: bathing, grooming, oral care; check skin integrity.
• Provide peri-care and Foley care and maintenance.
• Dress patients and help with transfers, turning, and safety.
• Take vital signs and weights as directed; assist with ambulation and ROM.
• Assist with medications ordinarily self-administered; follow care plan instructions.
• Document care in a timely manner per policies.

POSITION SUMMARY:

Under the direction of a Charge Nurse or Team Leader, provides quality personal care following an established Plan of Care and performs other supportive tasks as assigned.

DUTIES AND RESPONSIBILITIES:

• Assists with personal hygiene--bathes, grooms (hair and nails), provides oral hygiene, checks skin integrity.
• Provides peri-care and performs Foley care and maintenance.
• Dresses patients.
• Performs patient transfers, turns, etc. while ensuring safety of patient and self.
• Takes vital signs and weights as directed.
• Ambulates patients as appropriate and does range of motion exercise as directed.
• May perform simple dressing changes as directed by the RN (i.e. non-sterile, non-surgical, and not greater than Stage 1 pressure ulcers).
• Assists with medications that are ordinarily self-administered.
• Provides care consistent with written instructions as outlined in patient’s care plan.
• Demonstrates understanding and support in interactions with team members.
• Communicates with Charge Nurse and other team members as appropriate; reports changes in patient’s condition and needs.
• Documents care in a timely manner, and in compliance with organization policies and procedures.
• Obtains supplies and teaches use of medical supplies.
• Uses and reinforces the RN’s teaching on use of equipment.
• Checks equipment for safety and cleans equipment.
• Changes linens.
• Provides nutritional support, assists with feeding, light meal preparations and light housekeeping as indicated.

Qualifications:

• Current, active Texas nurse aide certification (CNA).
• Currently listed in good standing on the Texas nurse aide/misconduct registry, with no finding against the aide relating to client abuse or neglect or misappropriation of client property, as verified through the Texas Department of Human Services (DHS).
• CPR certification.

PREFERRED:

• Hospice or home health experience.
